iPhone 15 not Changing Volume With Buttons or UI

Why does the sound on my iPhone 15 not go to zero when I turn down the volume to zero on silent mode? The volume buttons and the UI both don’t do anything. I have repeatedly do the same action in order to get the desired effect. Please help.

Go to Settings > Sounds & Haptics > Headphone Safety. Turn on Reduce Loud Sounds, then drag the slider to specify the maximum volume.
