Actually, test this without any third-party (or even Apple) headphones, to rule out the connection and third-party devices.

Also, check if this is occurring only in a specific area or areas. It might be interference: Resolve Wi-Fi and Bluetooth issues caused by wireless interference - Apple Support

Can you isolate if this is only with calls to specific people, or every single call?

Can you isolate this is it occurs at a specific area of the house, or only at work, or while you are driving certain areas of the city or highway?

OR see if this occurs at a specific date and time regularly?
