Why is my kid’s Screen Time request not working correctly on their iPad?

My kids have downtime limits, but if they exceed their limit they can send a screen time request for me to approve. This feature has been working perfect except in the last week. They will send a request and I approve it, but the downtime doesn’t turn off. Twice now I’ve had to completely turn off the manage screen time option so that the app will unlock out of downtime. Then I’ve had to go in and reset everything.

i just looked at one of my child’s iPads and their request is showing as sent but no response even though I approved the request.
