my problem is that I wear a cochlear implant with Bluetooth installed and activated but call may be designated to choice between reg mic and speaker phone options. Very annoying and ineffective for communication

That's more an issue with the implant as opposed to your iPhone. I have at present, about 5 (maybe more) pairs of Bluetooth headphones connected to my IPhone. I have never needed to reactivate them to have a call automatically come through any of the headphones (a mix of over the ear and earbuds).
This is a question you should direct to the manufacturer of the implant for help with Bluetooth connection.
