Phone call and text forwarding issue on iPhone

I am concerned that my phone is forwarding texts and phone calls. When I dial *#21#, it says “setting interrogation failed, voice call forwarding on all calls”; “setting interrogation failed, data call forwarding on all calls”; setting g interrogation failed, fax call forwarding on all calls”.

when I dial the code ##21# that is supposed to stop it, it says “ setting erasure failed voice call forwarding on all calls”

can someone help to make sure my phone calls and text messages are not being forwarded
