When I try to change mr Ringtone. I get the Tick and can play the new Ringtone required.
Call my Iphone and I get the old ringtone.
Go Settings> Sounds Haptics> Ringtones can see my new ringtone is still ticked.
Net result is I am stuck with just the one ringtone.

Try restarting your iPhone first. If the new ringtone is still not working, go to Settings > Sounds & Haptics > Ringtone, reselect your desired ringtone, and ensure it's saved. If the issue persists, check for any pending iOS updates or reset all settings (Settings > General > Reset > Reset All Settings).
Many thanks. I already tried all the above but same problem.
Made a back up then completely wiped the phone.
Re-installed the back up as a new phone installation.
That sorted everything out ;-)
