Could the microphone / speaker / volume / be in use in a particular game or app.

or

You may want to try asking Siri to change the volume to see if that helps.

eg says:

"To turn the volume up or down, place your thumb on the stem of either AirPod and use your index finger to swipe up or down on the Touch control." (there's also an illustration in the above article).
