Apple Denial form to return the product delivered by amazon without otp

We have order iphone 14-128GB white colour from amazon and we requested on the same day for cancellation of the product. The customer care advised us not to share otp with delivery partner to get the order cancelled. Then we went out of station for some official purpose. when we returned, we found that the iphone was there outside out door step and was showing delivered on 22nd without OTP.

We have reached amazon many time for this issue.

Finally they have advise us to fill apple denial letter for return of the product.

Kindly help in providing this form to return our product.
