After updating IOS 18.4 the Apple Podcasts app is not removing played episodes. In Podcasts settings I have the "Remove Played Episodes" selected and this has always worked in the past.
Since this update, I now have to manually remove played episodes

I’m still having this issue with iOS 18.5 and iPadOS 18.5. My Mac is not on so I can’t speak to whether it is a problem there. It is not a problem on WatchOS 11.5.

Same problem here - AND also still a problem with 18.4.1 latest update.

iOS 18.5 should be out this coming week. This is a known problem and hopefully will be addressed soon.

Running 18.5, same problem persists.
I’m guessing that iOS 18.5.1 might be released this week.
Maybe a fix for this podcast bug might be included.
