Red/orange dot on Home Screen; Microphone icon in Dynamic Island

My iPhone 16 Pro Max now displays the microphone icon (orange) in the Dynamic Island, and I see a small red or orange dot in the upper right corner. I've gone through my settings and looked for anything that might be using the microphone or the camera. The little orange dot turns green when I launch the camera. Why do these icons and dots appear all the time? Do I have one or more apps using the camera or microphone? Should I be worried about security or privacy? I just need to understand what's going on. The iPhone 16 User Guide so far hasn't helped much.
