iPhones don’t store anything on the SIM card.
SIM cards provide cellular plan information only, they have nothing to do with your iPhone passcode.
If you locked your SIM card with a passcode and can’t remember it, you will need to contact your cellular provider.

No, a SIM contains no data, only carrier settings. Your passcode will not change.

No, but the new SIM may require what is known as a new PUK code. You would get that code from your carrier. This would have nothing to do with the passcode on your iPhone.
