Last night after updating the watch the Face ECG icon disappeared, I have it installed on my watch and can see it in the App View. The area circled in red is where the icon should be. I can click on the area and the app opens, but the icon is still black or disappeared.

I have the same issue. Changing the Sub Dial Top complication to something else seems to display its icon, and moving the ECG icon to another complication position appears to work. It’s a work around but not a proper fix.
I had the same issue. Tried restarting both iPhone and watch, making sure they were both updated, deleting all watch faces and reinstalling, scouring the internet for a solution. And then I inadvertently discovered that, even though the the ECG symbol isn’t visible, pressing the corner of the watch face where I’d installed it as a complication does indeed open the ECG app! Woot!
