IOS Home App not Syncing

I have a homekit setup with the latest Apple TV as my home hub, and the Home app sync well across my iPad pro, Macbook and iPhone 8. However, I have an iPhone 6 on IOS 12.5.7 (the lastest version that it can upgrade to), and its Home App is completely empty, no accessories or home settings are synced at all. It is just a starting screen with the add accessory button. Things that I've tried:

Turn off iCloud sync and re-enter the app to be prompted to sync.
Delete the app and reinstall.
Factory reset the whole phone.

None of them works.

Any ideas? Thanks in advance!

Is there a solution? Fairly frustrating when we all pay significant $$$$ and things suddenly stop working. Just mind boggling.

At a guess you have Upgraded to the new Home architecture - Apple Support, which leaves pre-iOS 16 devices behind as far as HomeKit is concerned.

Not that I am aware of. You can try Official Apple Support in case they can reset or delete any existing Home attached to your Apple ID so that you can use the older version of the Home app to set things up again from scratch.
