We'd like to share some helpful suggestions on determining your next best steps. We would recommend isolating the issue to determine if this only happens with this configuration. If you test the lighting adapter by itself and the Digital AV Multiport Adapter and verify they work independently, then this will isolate the issue to this combination. If this is isolation to just this combination, you'll want to reach out to Apple Support so this can be further investigated.
