What is using my location under a false name

An app called “surveys” pops up at the top of my phone saying it’s using my location services. I do not have any survey apps nor do I recognize the picture that the incoming shows. I’ve search my apps, the App Store, and my location settings. When I turn off my location setting it still shows the icon that states “surveys” is currently using my location devices. What app is doing this? How do I make it stop tracking me?
