It sounds as though FaceTime audio is muted when you start speaking on a FaceTime call. To confirm, are you able to hear that individual after you stop speaking? If so, let's start by restarting the iPhone using the steps specific to your iPhone model outlined in the following article. For reference, they're also listed below.

- "Press and hold either volume button and the side button until the power-off slider appears.
- Drag the slider, then wait 30 seconds for your device to turn off. If your device is frozen or unresponsive, force restart your device.

As for Audio/Visual not showing in Settings > Accessibility, it's possible that Content & Privacy restrictions may be enabled. If this setting is enabled, disable this feature in Settings > Screen Time > Content & Privacy Restrictions.
