I have the new Iphone 16 IOS 18.2 but do not have ChatGPT Extension to select (Apple Intelligence is on) on my phone.
Both my device language and region are the same and set to US English

Please check if that option is restricted in your Screentime:
Settings>ScreenTime>Content & Privacy Restrictions>Intelligence & Siri>ChatGPT Extension must be set to "Allow".
When done, restart your iPhone 16e.
