why does my music app create double or triple playlists?

My Music app creates a copy of whatever playlist that I create. Sometimes more than one copy. Then when I try to add music to my iPhone, it will delete some music. What's the problem?

Why is music showing downloaded on iPhone Apple Music when I did not choose that? When I tap on Downloaded in Apple Music, it lists quite a few playlists which I have NOT selected to be downloaded. (I have left Downloaded and gone straight to that playlist and verified that I have no chosen to download them.) Further, again under Downloaded, it lists many other playlists that have not been downloaded. When I tap into those playlists (under Downloaded) it may only list 2 or 3 songs from those playlists. I do not want playlists listed under Downloaded which have not been downloaded. I especially do not want playlists listed under Downloaded that I have not downloaded that only have a few songs listed. That is extremely annoying.
