How do I enable message notification tone for hearing aids in silent mode on iPhone 16 Pro Max?

My hearing aids are now not getting message notification sound in silent mode but I still get phone alert sounds in them (IOS 18.3.2). Tones are set for both in Sounds & Haptics and Play Haptics in Silent Mode is on. Yes receive notifications is on in Accessibility for hearing aids. Any suggestions?

I keep phone on silent mode so it doesn’t ring out loud but I’d like to hear tone for both in my hearing aids.
