Mac Podcasts App - How to Automatically Download New Episodes

The podcast app on my Mac does not automatically download new episodes. I've set the default to automatically download new episdodes. I've made sure I'm following podcasts. I've checked individual podcast settings to make sure they are set to automatically download new episodes. The app is running all the time. I'm connected to the internet. Even hitting /file/refresh feeds doesn't download latest episodes. The only way is to go to every show and manually download all the latest episodes.

Is there some secret to automatically downloading episodes?
