Syncing issue with Fitness+ app on iPadOS

I have a custom plan that I am using in Apple Fitness+. The scheduled workouts all sync between my iPhone, iPad, and AppleTV. However, if I complete a workout using my iPad, it does not show up in my plan schedule on my iPhone or AppleTV as being completed. And, any workout from my custom plan that I complete on my AppleTV or iPhone do not show up in my plan as being completed on my iPad. So it always looks like I have not completed my daily workouts in my custom plan. Syncing is working between my AppleTV and my iPhone. The issue appears to be with my iPad. All of my devices are on the latest version of the OS. I have already tried logging out of iCloud on my iPad. I’ve also reinstalled the Fitness+ app on my iPad. The only thing I have not done yet is wipe my iPad completely. I’m hoping to avoid needing to that if possible. Any suggestions?

I completely removed app. That didn’t work.

I wiped iPad and rebuilt from scratch (no from backup) and now the custom plan doesn’t even appear in Fitness+
