Unable to get activation lock disabled on iPad

I changed the Apple ID I was using on my iPad and can no longer access the ID. I factory reset my device and am now running into activation lock. I tried submitting the activation lock removal form but am getting denied. I filled out all of the information and provided the receipt but the are Saying I have not provided enough information. What more do I need?

iPad mini, iOS 9
