I have my junk mail settings set to delete the items in junk mail after 7 days. It used to work but now just sits and fills up indefinitely and those emails older than 7 days are not deleted. Tried to reset it, but nothing works.
Thoughts why this setting suddenly does not work?

It will delete them the first time I set it and then quit imail. I haven't left it for several days to see if it continues as tend to get a lot of spam filtered incorrectly so like to check it every now and then...
I can try this if the updates below don't work and just remember to scroll through before I quit.
As a shot in the dark, have a look at your mail settings to see if you have enabled "Store deleted messages on Server". If it is enabled, try unchecking that option.
Mail > Settings > Accounts.

The sad truth in my experience is Apple's junk mail filtering is just not that good.

I have the same issue with the latest Monterey OS. I can set delete when mail closes which works but no other setting does. I use to have it delete after one day but that no longer works.
