When I scan QR code with iPhone camera (iOS 16) it links to qr.io site which asks me to set up an account.
Is this malware or a change in Apple policy?

That is the link encoded within the QR code you are scanning.
This topic was raised last year but there never seems to have been an indication as to how to resolve it.
Basically if I scan a qr code with either the camera or the code scanner it takes me to the paid qr.io page even though I have never registered with that app.
I have an iphone 12 pro max running ios 17.5.1
I would really like to solve this.

It's most likely a phishing attempt, not malware. Do you want to create an account with qr.io? If you don't, delete the message with the QR code and forget about it.
