Unknown source accessing my iphone

My camera is being accessed by an unknown source. can you please assist me how to fix it?

I have tried resetting from privacy settings but it is still the same.

QR scan by apple camera links to qr.io When I scan QR code with iPhone camera (iOS 16) it links to qr.io site which asks me to set up an account. Is this malware or a change in Apple policy?

This is not an unknown source accessing your device.

An unknown contact is calling you on FaceTime or you are calling an unknown person.

Unknown: Not in the Contact list in you device.
