No. Everything that is in the backup will be restored. See here: Restore from an iCloud Backup - Apple Support

Get the App FROM the Developer, to Recover it:

iOS and macOS are two completely different platforms, which is why you may or may not find an app for both in the App Store. Go through the Developer, and you might have a chance to recover the data. You’ll find the Developer’s name and Contact Us info right in the app’s site of the App Store.

When backing up your iPhone, you can choose what data is backed up to iCloud/Mac. If you only want one app's data to be backed up, you can choose this in the backup settings!
