Lock Button to End Call Not Working
My dad is blind and needs to be able to end phone calls with the side lock button. However, it is not working. I checked the accessibility tab in settings and the “Prevent Lock to End Call” is not activated.

To get started, does your dad by chance have speaker phone enabled when he is trying to end the call with the Side button? If so, this would be expected.
To end the call with the side button, the audio would need to be switched from speaker to phone first.
If your dad does not have speaker phone enabled, has he tried restarting the iPhone since noticing the issue? If not, since restarting can often help with resolving some unexpected issues, give that a try with the steps shown below: Restart your iPhone - Apple Support
How to restart your iPhone X, 11, 12, 13, or later
1. Press and hold either volume button and the side button until the power-off slider appears.
2. Drag the slider, then wait 30 seconds for your device to turn off. If your device is frozen or unresponsive, force restart your device.
3. To turn your device back on, press and hold the side button (on the right side of your iPhone) until you see the Apple logo.

1. Press and hold the side button until the power-off slider appears.
