Screen Time keep resetting

I have 4 kids and use Screen Time to limit usage and have done this for a couple years. I just added an ipad pro and activated screen time on it. Within an hour or so, screen time is reset with no restrictions. I set it up again and it resets within an hour. I have tried the following: updated all devices to 16.3, restarted all devices, changed passwords, tried different settings, applied the settings directly on the ipad, applied the settings remotely using my phone yet nothing worked. I even kept the ipad on my desk doing nothing and the setting still reset within an hour to no restrictions. What the heck is going on?
