I would like to retrieve the Health App which was embedded on my iPhone XR.
The App has suddenly disappeared from the Home Screen.

Did you check, if it has been moved to the App Library?
Find your apps in App Library on iPhone - Apple Support
Also make sure, that no restrictions for the Health App are set in the settings for Screen Time, which would remove the app from your home screen as well.
