Is there a bug that will turn sound off and on at anytime and pause a video by itself?

Is there a way to turn off a bug that automatically stops volume anytime and another bug that pauses a video by itself

When I play a video in Safari, it has volume even though I turned the sound down completely prior to playing the video. Before the new iOS update, videos would remain silent if I muted the volume through the Control Center. However, with the latest update, playing a video causes the sound to turn on automatically, even if I've turned the volume down completely. Is there a setting or a fix so it does not change the volume on my phone?

audio in video cutting out with most videos ive taken in the past month or so, the audio goes out completely after the first second and then will come back 2-5 seconds later. any idea on how to fix this?

Videos mute after each replay After the current update whenever you watch a video replay (in the photos app) then escape one video and go to another one the next video is muted again. Before it would only mute if you exited the photo app, but now it mutes whenever you watch a new video. I have searched all settings and can’t find anything to change this and it’s quite frustrating to unmute after each view.
