Fall Detection by an iWatch series 6

I have an iWatch series 6 and an iPhone 11 Max Pro and the fall detection settings were long ago set up to do their stuff. I keep these two devices fully updated. I am also 82 and suffer from a form of arthritis. A few weeks ago I had a fall, my first, and serious enough to be x-rayed 3 times and ultrasound once, 2 doctors visits and a hospital inspection. Fortunately nothing was broken but it has taken a few weeks to recover stability and clear the bruises and head-rattling bounce. The issue is that the so-called Fall Detection did NOT record the event at all. Is this a failure of the iWatch, the iPhone, the WatchOS or what? Should I ask for my money back?
