Using one Apple TV account at two homes.

My spouse lives in another city during the week. Can we share one Apple TV account?

By "Apple TV account" do you mean an TV+ subscription or do you just mean an Apple ID / Apple Account?

Apple IDs should not be shared. You can have more than one Apple TV logged in to the same account, but that can get viewing history and other stuff mixed up between users and make things hard to watch.

If you mean a subscription, yes, you can share the subscription over Family sharing and each of you can get your own viewing history and watchlist without messing with the other user's viewing history.

Once family sharing is set up, services like the Apple TV+ subscription can be directly accessed on the other Tv logged in with the other Apple ID / Apple Account.
