After a chat with Apple support, the only solution was a Reset All ( Settings > General > Transfer or Reset iPhone > Reset > Reset All Settings)

They said Apple is aware of the issue and are working on a fix to be implemented in an iOS update.

I have an iPhone 16 and an iPad Pro, both running iOS 26.1. This option works on the iPad but not on the iPhone, despite trying all of the fixes above.  It’s annoying, but it’s important to realize that even having this setting set to Always Ask, this is not a true security feature. It does not evaluate the USB accessory, it does not warn the user against an identified risk. It just says WAIT! ARE YOU SURE YOU WANT TO DO THIS? Useful, perhaps, especially if you tend to plug in without forethought, but hardly essential.

Despite this insight, I am going to Reset Settings. The fact that the problem only exists on the phone makes me wonder about the phone. If it is a hack, I want to see if a reset will catch it.

Totally agreed, I’ve used iPhone since 2009 and loved it, but the pile up of bugs is becoming more and more annoying. I have addressed this in general to Apple, plus specific issues of course, that I hope they start to focus more on quality again, not only quantity, in their future OS updates. Not sure if it helps in the bigger picture, but at least I have tried to do my part in making the frustration known…

This topic is very relevant to me. I've been having this issue since the feature came to the public release, I have IOS 26.1 installed, I've tried the lockdown mode trick, which worked once, but then not, and so before I go and agonize as to whether to reset to factory and set up as new iphone, ... which will take me forever to tweek again, is it just reset all settings I should try? And if I back up before resetting, and then restore from backup, won't the problem persist? Please answer this question with a level of tech savvyness that you could use with someone who is not that tech savvy LOL. I'm totally blind, reliant on Voiceover to use my phone, so if I have to set up the phone like new and NOT restore from a backup, I'll be spending days retweeking the settings so it's usable again. When you have accessibility considerations, there' are a lot more tweeks necessary.

Also, someone mentioned in one of the posts about using Outlook being related to this setting. If that's so, and you have access to corporate stuff, does that mean you would literally have to use 2 separate devices to mitigate this effect? So like having one device with only personal email accounts and/or teams, and the other with all the corporately managed stuff? Because that's a great solution if you have a corporate phone, but it's not so great when you don't.
