This is frustrating.
Safari keeps deleting Reading List entries! It has also become unreliable saving when I “Add To Reading List”.
If I click on a Reading List entry I have managed to save, it then disappears! I’m on iPad OS 18.6.2 and this has only started recently!
Any ideas!
I believe reading list entries are saved as cookies, so if you are deleting cookies, or have a setting to automatically delete cookies you will lost entries saved in them.

I missed that you were on an iPad. How many pages do you have open? Tap on the boxes in the upper right. If you have more than one tab group check all of them.
It happens irrespective of the number of pages open.
