The item declined by my bank, and I cancelled it. But it shows in my subscriptions as pending and I can’t download anything else because of it. How do I get rid of it without paying $39.99 and asking for a refund. I can’t really afford that.

If your bank refused to pay the charge, your Apple Account is in default, and Apple has frozen it.
Until you contact Apple and settle the situation, your account will remain frozen.
