i am trying to copy a photo into the notes app from my ipad to my phone and vice versa. the file does show up however, the format is [image_####.thumb.low] and it does not show the photo immediately and can't be opened as well.

Did you use Share>Copy in the Photos app, or something else? It looks like you may have used a method that doesn't really copy the picture.
As a troubleshooting step, have you tried to past the picture into Notes on the phone to eliminate the handoff as a source of the problem?

yes i have tried that. i also tried using handoff on the phone and paste it in the notes app of the same phone and only then it shows the photo.
