If a playlist was synced from iTunes to the phone, and then updated on the phone, those changes should sync back into iTunes at the next sync. This assumes that all works as it is supposed to, and that there haven't also been changes made in iTunes or any other reason for iTunes to decide the list in the library should overwrite the copy on device. There is seemingly no feature for conflict resolution. In principle as long as you don't turn on the feature to sync automatically on connection, and the content in your device has been synced from iTunes, then it should be possible to export a playlist that exists on the device as an XML file, then import that XML file after syncing to recreate the playlist in iTunes the way that you had it on device. Or you can try my script ImportDevicePlaylists.

In addition to turingtest2's suggestions, I suggest that you ensure that you are still using Sync and not Manually Manage. and that you have the appropriate Sync options ste. It''s not uncommon for people to say think are using Sync when they are in fact using Manually Manage.

Playlists updated on the phone are only copied back to iTunes by a Sync.
