Old iPhone, never set up apple account or iCloud for the device just started using it. Now years later I have forgotten the four digit passcode so it’s locked and as I understand I can’t access it at all. Any options or is all data unrecoverable?

If you don't remember the passcode, you'll have to erase the old phone to get back in. That will erase all the data on the phone. If you were backing the phone up, you can restore the backup after getting back in. If you weren't, the photos you want will be gone.
