On the new iPhone 16 my alarm keeps resetting to maximum volume (incredibly loud) every few days. I’ve gone to ringtones (sounds and haptics) and adjusted it back each time and disabled the ‘Change with buttons’ option (also tried with it enabled) but still it persists.

Thanks for the kind response. I believe the use case the other user is reporting is different. For me it’s the alarm so I’d like it to be permanently fixed at a lower volume rather than having the ability to reduce the volume when it does happen (because it wakes me from a deep sleep the sudden loud burst of sound is really bad).
I checked my settings for ‘reduce loud volume’ and that’s switched to on already.
