Apple Devices App not syncing all photos from PC. PC folders lost during sync. Nothing in 'From my Mac'.

MULTIPLE ISSUES THAT I WOULD LIKE RESOLVED: When using Apple Devices App on PC to sync photos from Windows PC to iPhone ... 1) Not all photos copied; Most recent not synced; 2) Sync has lost all sub-folder structure (PC sub-folders organized by Year Month) and now there is just have a blob of photos without any logical organization on my iPhone and 3) Nothing appears on iPhone/Albums/From my Mac.
