Talk to Text Cuts First Few Words

When I do talk to text using Siri, the first few words get cut off. I can see they are typed on the screen, so the words are heard, but then they disappear. Also, if I state a few sentences, including punctuation, it will remove the first sentence entirely and only capture the last half of the final sentence. I have an iPhone 16 running 18.3.1. It did this on my iPhone 14 and has only gotten worse on the 16. Also, on the latest iOS Siri has stopped saying, "what would you like to say to so-and-so" so I'm concerned she may be sending to the wrong person, and I can't get Siri to speak from the speaker to say, "ready to send?" so I don't know to say "yes."
