Why won't my Mac recognize 4x6 paper size?

I have set up a thermal 4x6 printer. No matter what print option or app I choose to custom make a 4x6 page, the MAC always thinks it's printing 8x11. The 'standard' defaults do not contain 4x6 or an allowable section to add a default for the printer.

Choosing 4x6 each time still does not do the trick.

I am on Sonoma 14 version of a 2023 or 24 macbook air.

Printer driver is up to date

I am going through labels like crazy testing all the help and youtube guidance out there.

I am clearly missing something to set up my thermal label printer to tell mac it's a 4x6 page.

has nothing to do with that. Let me phrase another way. MAC won't keep a default setting of 4x6 in the default paper size. So each time I select 4x6 to print to the thermal print, MAC overrides and chooses 8.5x11. rather annoying . I'm not the first person to print from a mac to a thermal so there has to be a setting someone can help with. I tried mac support,they were useless last night.
