My iphone 12 pro max camera has stopped focusing when zooming past about 2.5. Sometimes it has a second in-focus, then it is blurry. Since it is in-focus for a small time, I feel it is a software glitch, not a lens issue.

I have no problem zooming in on something close - I can zoom to 12x and it is still in focus. It is when I zoom on something that is distance that it goes blurry after about 2.5 times zoom. I just tested it again, and it was blurry. I left the zoom on 12x , moved my phone around - some things closer - then lifted it up to the distant object again. It was in focus for a very short time, then went blurry again.
Most likely you are too close for the camera to focus. See if it clears up if you move further from your subject.
