Missing "Silent mode" in Control center in iOS 18

I have recently decided to clean up my control center, delete everything and start from scratch. However, when it came to "Silent mode on/off switch" I could not see it in the list of available options. Perhaps, I am missing something, but it seems that this option was not added.

Trying to understand the difference between "Silent Mode", which I turn on with the little switch in the upper left hand corner of my SE, and Focus, both of which are intended to keep the thing quiet. If I just want to keep the thing quiet, which one should I use? They are both a little annoying in that "Silent Mode" doesn't seem to offer any indicator (in Settings, for example) that it's on, and Focus, in the Control Center, just says "Focus", when it's off, and only says that it's on when it's on.

How can I prevent Silent Mode from disabling automatically on iPhone 16 Pro? After turning on Silent Mode using the Action Button on my iPhone 16 Pro, silent mode will automatically turn off after some time. I am certain that I'm not pressing the Action Button accidentally or disabling Silent Mode any other way manually. I don't use any Focus modes, nor do I have any focus modes set up to activate automatically. This has been occurring on all versions of iOS 18, up to and including the most recent 18.3. It is incredibly frustrating when I get a call blasting at full volume when the phone should have been silent. Any help would be much appreciated. [Re-Titled by Moderator]
