If you live in the US and order online, you will need to mail in your old phone in a box Apple will send you and you have up to 14 days to mail it to Apple's trade in partner company. They will determine the final trade in amount/credit.
If you buy it in a store, you can take your old phone and trade it in during the purchase process.
And no, you cannot take the trade in phone to a store in another country.
