What happens to my AppleCare account when I upgrade my phone

I have upgraded one of my phones, how do I transfer my apple care to my new iPhone

You don’t. AppleCare is associated with a device, not a person.

You can cancel your apple care plus and get a refund for the unused months, if you paid upfront or cancel the monthly payments and then when you get the new iphone you purchase a new Apple care plus
