iPhone 14 Plus was bought less than a year ago only uses an original charger of course the health of the battery has dropped to 89% the first time this has happened to me requires a battery replacement because it is still under warranty

This is an approximation, that is why it may be plus or minus a few percent on a month to month basis.

But this is the first time I encounter such a problem, it also gets really hot for me while charging. This is not my first iPhone, I have had several iPhones and the battery health has never gone down very quickly
I tried but all the time it transfer me to the Apple support app and when a chose an issue for example- battery service it’s only doing a check of my battery performance.
How can I schedule an appointment? It only gives me in my country and not in Italy. Only if I'm there will I be able to do then?

You have to go through the diagnostics first. If they determine your battery needs to be replaced, you'll be taken to the appointment screen.
