Currently complications are limited on how many times per hour they can update. If a complication were permitted to update constantly, battery life would be severely reduced. The compromise to satisfy battery life with also update a current complication is to tap the complication and then open the app to see a current total or have a current reading done. The limit on complications is usually 6 updates per hour.
On a side note, doctors are starting to deemphasize number of steps as a fitness measure.

Currently there isn't a complication to add steps to your Apple Watch face, however you can view your daily steps by following this path:

You’d think with “steps” being a metric everyone is talking about this would be a complication option. I care less about standing and more about steps.
