i keep getting ads for happy hospital asmr whilst playing other mobile games and the ad for this game makes me feel physically sick, change the advert or just don’t advertise such a disgusting game, there should be a way to block certain ads someone needs to do something about this.

If you have received an invoice from Apple, tap on Report a problem on the invoice.
Then you can choose an option:
OR see -> Find the URL for an app in the App Store to report a concern
Find the URL for an app in the App Store to report a concern - Apple Support
Only the app developer can really do anything about that. They need to talk to whoever it is they contract with to serve the ads to not include that one. Complain to the developer.
I saw that ad a few times in some game a couple of weeks ago. But only on one day. Disturbing though you and I both find that ad, I don't think it violates any of Apple's rules.

You can't. Just because you don't like the ad doesn't mean that there's legal justification for restricting it. You can stop using the game and complain to the developer.
