pop up warning on macbook that icloud has been hacked

I keep getting a pop up on my MacBook stating my iCloud has been hacked. What do I do?

MacBook Pro 13″, macOS 10.12

Fake security alert banners on my MacBook Pro Had Banner Pop Ups appear stating that someone hacked my account (iCloud, Bank). Funny is that I do not have an account with BOA which was the first clue that this is nefarious. Unsure what App is initiating this prompt. Any ideas of origin and how to remove. Clear all history and website data Clear your browsing history in Safari on Mac - Apple Support

If you are using safari for browsing , click on the url link and use the settings as per screen shot .
